The Loyalty Program for CandidPro Products (the CandidPro Loyalty Program) allows Practices to earn points and associated benefits as they advance through each Loyalty Tier. The points and applicable discounts are described under the Loyalty Section in the CandidPro Portal or the Help Center and the CandidPro Loyalty Program Flyer delivered to you. These CandidPro Loyalty Program Terms and Conditions further describe the terms and conditions applicable to the CandidPro Loyalty Program.
1. Program Period. This Program is effective from April 15, 2026 (or March 3, 2025 for new customers that sign CandidPro Agreements starting on such date). Your loyalty qualifying period will end every 6 months on the 14th of the month—instead of the date you joined as a provider. Your new qualifying period reset date will be “rounded up” to the closest 15th of the month. For example, if your qualifying period date was May 8, it will now be May 15 and if it was May 19 it will now be June 15.
2. Enrollment. Practices are automatically enrolled in this Program by Candid. Practices may terminate enrollment at any time and for any reason by sending an email with the subject line “Opting Out of Loyalty Program” to [email protected]. Opting out of the Loyalty Program will take effect on the first day of the next Program Period, at which time the Practice will forfeit all Program points, Tier designation, discounts, and other benefits. The Practice may re-enroll at any time and will begin earning Program points, discounts, and other benefits from the time of re-enrollment. 3. Points.
a. Purchases of the following Points-Qualifying Products allow Practices to earn points under this Program: CandidPro Comprehensive, CandidPro Elements and PermaForm Retainers.
b. Points are earned by the Practice every time a Practice approves a treatment plan for a CandidPro Comprehensive Case or CandidPro Element Case. For PermaForm retainers, points are earned only when (1) the Practice submits a retainer case and (2) Candid reviews and approves the submission. In the event of order cancellation after a treatment plan is approved, the corresponding loyalty points will be deducted from the Practice’s Loyalty Program account.
c. The number of points earned for each purchase of a Qualifying Product is described in the CandidPro Loyalty Program Flyer and the Loyalty Section in the CandidPro Portal or the Help Center.
d. Points earned under this Program have no cash value, cannot be redeemed for goods or services, and may not be transferred or assigned.
4. Loyalty Tier. A Practice Loyalty Tier is determined by Candid based on the number of points earned by the Practice during the prior Program Period, except that a Practice may advance to a higher Loyalty Tier –and be awarded with the benefits of the higher tier – at any time once it achieves the required points.
5. Discounts. Discounts under the Loyalty Program apply, and may only be used, for the purchase of Comprehensive Cases. The amount of each discount is based on the Practice’s Loyalty Tier, which can be found under the Loyalty Section in the CandidPro Portal or the Help Center and is described in the CandidPro Loyalty Program Flyer.
6. Other Promotions and Discounts. (a) This Program may not be combined with other discounts or promotions unless Candid expressly states otherwise in writing prior to the Practice’s order. In addition, the aggregate maximum discount shall be limited to the highest Loyalty Tier discount available under this Program, unless Candid expressly states otherwise in writing prior to the Practice’s order.
7. Definitions:
“Candid” means Candid Care Co.
“Loyalty Program Flyer” means the document sent to you by email.
“Practice” means a dental practice that has signed a CandidPro Agreement with Candid and is not in breach of any of its terms.
8. Confidentiality. These Loyalty Program Terms and Conditions and the contents of the Program are confidential and must be protected as such and may not be disclosed to any third party without Candid’s prior written consent, except to the extent disclosure is required by law.
9. Obligations of the Practice. By participating you represent that your participation does not violate any third-party rights or policies. Each Practice and its doctors has responsibility to comply with all applicable laws, regulations and reporting obligations, including reporting the discount under the Loyalty Program to any governmental healthcare program that may provide reimbursement, to the extent applicable.
10. Candid may, from time to time, amend the terms of this Loyalty Program and any of its contents, including availability, discounts and other benefits, upon prior notice to the Practice.
11. If a Practice of any of its Doctors commits fraud, the Practice's eligibility to participate in this Loyalty Program shall be terminated, and the Practice will forfeit any Loyalty Program benefits.
12. Candid reserves the right to request additional information from you at its discretion.
13. These CandidPro Loyalty Program Terms and Conditions and the contents of the Program shall be void to the extent prohibited by law. If any material term of these CandidPro Loyalty Program Terms and Conditions is prohibited by law, then either Candid or the Practice may terminate the Practice’s participation in this Loyalty Program with immediate effect and without penalty.
